PT Plus in Milwaukee’s Historic Third Ward is a full-service physical therapy and occupational therapy clinic located on N. Broadway, just south of downtown. We serve patients from the Third Ward, downtown Milwaukee, Walker’s Point, and the surrounding neighborhoods. Whether you’re recovering from an injury, managing a chronic condition, or getting back to the activities you love, our licensed therapists are here to help you move better and feel stronger.

We offer flexible weekday hours and personalized one-on-one care, because your recovery shouldn’t have to wait.

Your First Visit

Not sure what to expect? Here’s how it works:

  1. Schedule your appointment — Call or text us at (262) 796-2850, or use our online scheduling tool to pick a time that works for you.
  2. Complete your intake forms — We’ll send these ahead of your visit so you’re not filling out paperwork in the waiting room.
  3. What to bring — Insurance card, driver’s license, physician referrals or medical records.
  4. Meet your therapist — Your first session includes a full evaluation: we’ll review your history, discuss your goals, and assess your movement and pain.
  5. Get your plan — Before you leave, you’ll have a clear, personalized treatment plan with a timeline and next steps.

Plan for about 60 minutes for your initial evaluation.

By car: We’re located on N. Broadway between E. Buffalo St. and E. Chicago St., in the heart of the Historic Third Ward. Metered street parking is available on Broadway and the surrounding streets, and several public parking structures are within a short walk.

By bus: The Milwaukee County Transit System Routes 14 and 15 stop within a few blocks of our clinic. Plan your trip at RideMCTS.com

By streetcar: The Milwaukee Streetcar (The Hop) has stops at Cathedral Square and Historic Third Ward — both within walking distance.

From downtown Milwaukee: Head south on N. Broadway or N. Water St. We’re just a few blocks south of Wisconsin Ave.

From I-794: Take the Plankinton/2nd St. exit and head south to E. St. Paul Ave., then east to N. Broadway.

From the East Side: Take N. Prospect Ave. south to E. Wisconsin Ave., then south on N. Broadway.
