PT Plus in Bay View is a full-service physical therapy clinic located in the Lincoln Warehouse on S Chase Ave., serving Bay View, Tippecanoe, St. Francis, and Milwaukee’s south side neighborhoods. Whether you’re recovering from an injury, managing a chronic condition, or getting back to the activities you love, our licensed therapists are here to help you move better and feel stronger.

In addition to physical therapy, our Bay View location is home to a wellness center offering integrative practices, including massage, Reiki, reflexology, Thai massage, somatic coaching, and breathwork, along with a movement studio hosting yoga, Nia, drum circles, children’s music circles, and grief support circles.

We offer flexible weekday hours and personalized one-on-one care, because your recovery shouldn’t have to wait.

Your First Visit

Not sure what to expect? Here’s how it works:

  1. Schedule your appointment — Call or text us at (262) 796-2850, or use our online scheduling tool to pick a time that works for you.
  2. Complete your intake forms — We’ll send these ahead of your visit so you’re not filling out paperwork in the waiting room.
  3. What to bring — Insurance card, driver’s license, physician referrals or medical records.
  4. Meet your therapist — Your first session includes a full evaluation: we’ll review your history, discuss your goals, and assess your movement and pain.
  5. Get your plan — Before you leave, you’ll have a clear, personalized treatment plan with a timeline and next steps.

Plan for about 60 minutes for your initial evaluation.

By car: We’re located inside the Lincoln Warehouse complex on S Chase Ave., just south of E Lincoln Ave. On-site parking is available in the building’s lot.

By bus: The Milwaukee County Transit System Route 15 (Oklahoma–Holton) and Route 51 (Lincoln–Forest Home) stop near our location. Plan your trip at RideMCTS.com

From Bay View: Head south on S Kinnickinnic Ave., then west on E Lincoln Ave. to S Chase Ave. We’re just a few minutes from KK and the heart of Bay View.

From downtown Milwaukee: Take I-794 south to the Lapham Blvd exit, then head south on S 1st St. and east on E Lincoln Ave. to S Chase Ave.

From St. Francis or Cudahy: Take S Kinnickinnic Ave. or S Howell Ave. north to E Lincoln Ave., then west to S Chase Ave.
