Your First Visit

Not sure what to expect? Here’s how it works:

  1. Schedule your appointment — Call or text us at (262) 796-2850, or use our online scheduling tool to pick a time that works for you.
  2. Complete your intake forms — We’ll send these ahead of your visit so you’re not filling out paperwork in the waiting room.
  3. What to bring — Insurance card, driver’s license, physician referrals or medical records.
  4. Meet your therapist — Your first session includes a full evaluation: we’ll review your history, discuss your goals, and assess your movement and pain.
  5. Get your plan — Before you leave, you’ll have a clear, personalized treatment plan with a timeline and next steps.

Plan for about 60 minutes for your initial evaluation.

By car: We’re located on S. Green Bay Rd. (WI-31) just south of Washington Ave., in the Mt. Pleasant area of Racine. Free on-site parking is available in the building’s lot.

By bus: The Racine Transit System (RYDE Racine) Route 27 runs along Green Bay Rd. and stops near our clinic. Plan your trip at RYDERacine.com

From downtown Racine: Take Washington Ave. (WI-20) west to S. Green Bay Rd., then head south. We’re a short drive from downtown.

From I-94: Take exit 333 for WI-20/Washington Ave., head east to S. Green Bay Rd., then south. We’re just a few minutes from the freeway.

From Kenosha: Take I-94 north to exit 333, then follow the directions above. We’re about 20 minutes from downtown Kenosha.
